This class will teach person-centered thinking, planning, and service delivery approaches, focusing on Personal Futures Planning, MAPS and PATHS. The course will provide 20 hours of instruction and practice. Participants must complete two person-centered plans in order to earn a Certificate of Completion. This course is supported by Senior and Disability Services (SDS) with the State of Alaska, and may count toward ongoing certification or re-certification for Care Coordinators.
Person-Centered Thinking, Planning & Services
A distance delivered course to teach and certify Person Centered Planning Facilitators offered by UAA Center for Human Development.
In order to take this course, PARTICIPANTS MUST HAVE:
- A current functioning computer with a webcam (required)
- A headset with a microphone (preferred) or computer speakers/microphone
- Access to high speed internet connection
The course will provide 20 hours of instruction & practice. Participants must complete two person-centered plans with instruction and technical assistance in order to earn a Certificate of Completion.
This course is supported by SDS, and may count toward ongoing certification or re-certification for Care Coordinators.
Cost: $125.00 covers materials, fees, and instruction.
The class will meet on Wednesdays from 9:30 AM – 1:30 PM AKST April 17 through May 29, 2019.
